Faith-Based Senior Living Communities
The Communities We Serve
From our humble roots serving the sisters of St. Joseph, CSJ Initiatives has grown to now service 12 communities in seven states across the country, including:
- CSJ Rocky River, Cleveland, Ohio
- Joseph House, La Grange Park, Illinois
- Hundred Oaks, Baton Rouge, Louisiana
- Nazareth, Kalamazoo, Michigan
- Mount St. Joseph, Wheeling, West Virginia
- Mount St. Mary, Wichita, Kansas
Our dynamic model of life-giving care, called “Relationship-Centered Living,” integrates the needs and values of those we care for in a manner that honors the individual preferences in a homelike environment. Our approach and experience uniquely enable us to respond to the emerging needs of religious congregations and organizations.
As our network of sisters, communities and management groups expands, our services are able to help more organizations achieve their goals.

Relationship-Centered Living
Impelled by the Mission of Jesus, “that all may be one,” drawn by His example of uniting love, we live and work guided by these principles:
Respect each person’s dignity, uniqueness and spirituality
Embrace a culture of healing presence for mind, body and spirit
Empower each person toward positive health and wellness
Encourage each person’s ability for self-determination and purposeful living
Honor and respect the voices of all who live and work in this community
Recognize all behaviors as opportunities for wholesome communication and relationship-building
Cultivate and nurture interrelationship with Earth: source of life, renewal and healing
Celebrate what is right with the world
